Your First Visit
If you are a new patient, we request that you arrive at least 30 minutes early to help us establish your record. New patient information forms will be completed upon your arrival to the clinic. (If you prefer - All forms can be downloaded, printed, and completed prior to your visit. Select the appropriate form from the Forms link on the left sidebar.)

Established Patients
If you are an established patient, we request that you arrive at least 20 minutes early. Bring your insurance card with you to every visit.
Telephone Calls
Your telephone call is important to us. The following information should be given when calling the clinic to assist the staff in expediting a resolution for you: Your name, Telephone Number, Date of Birth, Physician’s Name, and Purpose of the call.
The nurse that takes your call will talk with your physician concerning your problem and will return your call; unless, the physician thinks a direct conversation is needed regarding your question.
Phone calls dealing with routine questions such as insurance billing, appointments, and prescription refills, can be handled by the office personnel and should be made in the morning when the phones are not so busy.

Prescription Refills
Refills for prescriptions written by Gastroenterology Associates physicians should be requested by having your pharmacy fax a refill form to our office at 501-664-4738. Requests should be made at least one week before your supply is gone.
